This may include various forms (informed consents, HIPPA forms, etc., release of information, etc.); papers on documentation strategies, and other information which may be useful to depth psychotherapist.

We also hope to provide useful resources for marketing and developing your practice. This may include papers and webpages submitted by practitioners. See the submission guidelines if you are interested in submitting a paper or webpage.

Please note each individual clinician is responsible for developing their own forms and making sure that all forms and recourses are compliant with applicable laws, regulations, and ethics codes. Neither the Depth Psychotherapy Network or the contributors are responsible for appropriate usage of forms. These forms are offered as a stimulus to give clinicians ideas for developing their own forms. It is not recommended to use these forms as a stencil.
